HOW IS THE CONFIDENTIALITY OF INDIVIDUAL RESPONDENTS AND SUBSCRIBERS PROTECTED?

Individual uSPEQ® respondents are assigned codes for data entry. This code is not linked to information identifying the respondent at the subscriber organization.

uSPEQ respondents cannot specify any identifying information on the questionnaire, such as a name, address, social security number, medical record number, or contact information. As with respondent questionnaires, organizations are identified by codes to protect their identity and confidentiality.

HOW ARE DATA PROTECTED?

uSPEQ has established specific precautions for confidentiality and security. The uSPEQ application and database reside on servers reserved for uSPEQ. The servers are secured physically and at the computer file level and are protected with industrial firewall security software. Access to uSPEQ servers and resources is audited, and that information is stored in a database. In full compliance with applicable regulations, procedures are in place to ensure the privacy and security of individual respondents. Access to paper and computer files is limited to uSPEQ staff and protected by both physical and electronic locks.

ARE MY RESULTS VIEWED IN THE ACCREDITATION PROCESS?

No. There is a firewall between uSPEQ and the CARF accreditation process. Your results or participation in uSPEQ has no effect on accreditation. However, subscriber organizations may decide to incorporate the uSPEQ results into the accreditation process.

HOW DOES uSPEQ PROTECT CONSUMER DATA?

uSPEQ protects the privacy and security of consumer data consistent with applicable federal, state, provincial, and territorial privacy and security laws.

In connection with the uSPEQ Consumer Experience Survey, uSPEQ enters into a business associate agreement with entities covered by the United States' Health Insurance Portability and Accountability Act (HIPAA).
